if the child is 130lb and 4"8 can he wear a child large costume

He may be ok. There is room for the blow up belly that is included in the costume, so if anything, just skip the belly!

I have an 8 year old who is 60lbs and 52 inches tall (tall and thin). Should I get a medium or a large?

I would say a large if he is tall the costume has some room on the belly part so you can Inflate the fake belly that comes with it my son wore it last year and he was 7 mine is more on the short side and I got medium.

Does this costume include the yellow shirt?

It is a one-piece suit that includes the yellow chest and arms.
